Whether you are a new or established patient, please contact us as far in advance as possible to best schedule a date and time that works well for you. Existing Patients:

Please either call us for assistance in scheduling over the phone, or conveniently use the My Appointments feature on our patient portal and follow these steps below:

  1. Login to the patient portal using your provided username and password.
  2. In the toolbar section at the top of the page, select “My Appointments”.
  3. In the left side of the page select “Request”
  4. Click the “+New” button and then provide and submit the required information

Note: If you are an existing patient but do not yet have a portal account, please give us a call and we’ll be happy to send you a registration invitation. We only need your full name and email address to get you started.

New Patients:

If you are new to Lac Vieux Desert Health Center, please call us at either of the locations listed at the bottom of this page. We’ll be very pleased to assist you with making an appointment.

Please note that we request that all new patients complete a New Patient Registration packet. This packet can be collected and completed at one of our locations, or you can download a copy from the link below.
